Completing it earns Kroger’s 50 Fuel Points Survey Bonus, limited to one per 7 days.

How to take the Smith’s survey
- Open smithsfoodanddrug.com/feedback, which redirects to the Kroger Customer Satisfaction Survey.
- Pick English or Spanish.
- Enter the Entry ID from your receipt, or choose “No” and enter your visit’s date and time instead.
- Answer the questions about your trip.

Questions about the Smith’s survey
Is this the same survey as kroger.com/feedback?
Yes. On 9/30/26 we opened smithsfoodanddrug.com/feedback and it sent us to the same Kroger Customer Satisfaction Survey form.
Do I have to log in?
No. The form asks whether your receipt has an Entry ID, and if it doesn’t you pick the date and time of your visit.
What do I get for it?
Kroger’s survey page still says “Limit one 50 Fuel Points Survey Bonus per 7 days”. Kroger renamed its Fuel Points program to Points in June 2026, but the survey page kept the old name, so the wording could change. Our Points guide explains how the program works now.
What if I lost my receipt?
The form asks for details from your receipt. Read our page on entering the survey without a receipt before you start.
Where is the pharmacy survey?
The form is titled “In Store or Pharmacy”, and there is a separate pharmacy survey button further down this page.
