Generally, krogerfeedback survey questions are divided on these categories:

Questions asked in these sections are typically in the form of rating, where you have to rate your experience from worst to best. You will see a questions followed by radio button under heading divided in to 5 satisfactions levels -> Highly satisfied, satisfied, neither satisfied nor dissatisfied, dissatisfied, and HIghly dissatisfied.

Second Type of Questions will be purely based on your purchase and you will be given a multiple choice format to answer each questions. You will be allowed to choose more then one option in these questions. i.e Which of the department did you visit or made a purchase form: Meat, Fresh produce, Dairy,  Other. If you choose Other then you will pre required to manually enter the department you visited.

If you used your Kroger Plus Card, the survey may confirm:
